Medical Records Request

In partnership with ChartRequest, we are happy to provide you with your records quickly in a HIPAA compliant way. ChartRequest offers a simple, efficient, and secure solution to provide your records electronically. Please follow these steps to submit your request.

SIGN UP/LOGIN

• If this is your first time using ChartRequest, select “Sign Up” to create your account (Please note, you will need to upload an image of a government ID)
• If you have an account, select the state and location of the practice you visited and click “Log in”

OPEN A NEW REQUEST

• Navigate to your “Dashboard” in the left hand menu (you should be brought here automatically upon login)
• Select “Records – Request Medical Records”

COMPLETE THE REQUEST

Follow each step and select/add the appropriate information, you will need to know the following:

• The state and facility you were seen at
• The dates of service you would like to request records for
• Reason for your request (i.e. change of insurance coverage, medical malpractice, moving/transfer of records, etc.)

Once your request is submitted, you will receive email updates on the status of your request. You can also login to ChartRequest and review your recent information requests to review status updates.

IMPORTANT NOTE – YOU WILL NOT BE CHARGED

If you are a Dermatology Center of Indiana patient you will not be charged for your request.
